IF YOU DON'T HAVE  (OR CHOOSE NOT TO USE)  INSURANCE BENEFITS,  PERFORMAX PHYSICAL THERAPY OFFERS OUR PATIENTS A 'SELF-PAY' RATE OF $100 FOR THE INITIAL EVALUATION, AND $80 FOR EACH ADDITIONAL VISIT.

Our office will contact your insurance provider prior to your first visit to verify your Physical Therapy insurance benefits.  The information we receive from your insurance company, however, is not a guarantee of benefits, and if a balance is owed after your claims are paid by your insurance provider(s), we will bill you for the balance. 

 

Ultimately, knowledge of your insurance benefits is your responsibility, but we will do whatever we can to assist you in the payment process.  Due to the overwhelming number of insurance contracts, we must rely on you to know the limitations and exclusions of your particular coverage. We strongly recommend you contact your insurance company or plan administrator if you are unsure of your physical therapy benefits.

 

Insurance co-pays/co-insurance is due at time of visit.  We accept cash, check, MC, Visa and Discover.
